For people who are homeless and have a disability, our Shelter Plus Care program helps provide both housing and support services.
Home Forward has partnered with several non-profit agencies to provide housing plus a wide range of support services for people with disabilities who are homeless. To be eligible, the head of the household must be experiencing homelessness, be an adult with a permanent disability, and be referred through our community's Coordinated Entry systems.
